*NO FAXED ABSENTEE APPLICATION REQUESTS ARE PERMITTED BY LAW

1. Your printed name and voting residence.
2. Your contact information. We will use this information to contact you of any errors that may have occurred.
3. Your HOME address at which you are registered to vote. (no P.O. Boxes or polling place address)
4. The mailing address to which you want your ballot sent.
5. Your date of birth.
6. Form of ID last four of Social Security Number or Driver License Number.
7. The election for which you are requesting a ballot. (only 1 election per application)
   • For Primary Election Only: Your party choice, if the election is a Primary, or that you want to vote an issues-only ballot. (Under Ohio Election Law, the ballot you vote in a Primary Election determines your party affiliation).
8. You must read and agree to:
   • I understand that, per Ohio law, the board of elections must receive this request no later than the close of business seven days before Election Day.
   • I understand that if an absentee ballot is mailed to me and I change my mind and go to my polling place to vote on Election Day, I will be required to vote a provisional ballot.
   • I understand that if I do not provide the board with all of the required information, my application cannot be processed.
   • I hereby declare, under penalty of election falsification, that I am a qualified elector and the statements above are true.
   • Your complete legal signature or mark.
   • Ohio Election Law does not recognize a Power of Attorney. If the voter is unable to sign, the voter must make a mark and may be witnessed and signed by another person.
9. Consider working as a Poll Worker on Election Day

Note:

• All timely received absentee ballots are counted. (Make sure your ID Envelope is completely fully and correctly.)
• The absentee ballot is the same ballot you would receive at the polls.
• The absentee ballot results are the numbers you see on Election night at 7:30 on our web site.
• If you request an absentee ballot and decide to vote on Election Day instead, you will be required to vote provisional ballot.
• If voting in person, you may apply at the Board of Elections during regular office hours up to Sunday before the election.
• If requesting a ballot by mail, your written request must be received at the Board of Elections by the close of business on the 7th day prior to the election for which you are requesting the ballot.
• Please request an absentee ballot as early as possible and allow enough time if returning ballot by mail delivery to meet the return deadline.
• All voted absentee ballots must be postmarked by the day prior to the election (and received within 4 days after the election), or hand delivered to the Board of Elections office by 7:30 p.m. on Election Day in order to be counted.
• Ballots mailed from out of the country will be counted if received by the Board of Elections within 4 days following the election if post-marked by Election Day. If you have questions, please call the Board of Elections at 513-695-1358.

Absent Voter Ballot Application - Medical Emergencies

In the case of medical emergencies, the absentee ballot request deadline is 3:00 p.m. on Election Day. To be eligible under the provision, you or your minor child must be admitted to a hospital after 12:00pm Saturday before an election.
